Traditional Targeted Case Management (TCM) is a mental health service that addresses mental health-related needs that are outside the scope of mental health counseling such as housing, food availability, clothing, school supplies and more. Individuals and families need to have these basic needs in order to best focus on and address their overall mental health and wellness. Our TCM services link families to, coordinate and advocate for their basic needs services.
What sets Tykes Teens’ TCM services apart is that our case managers provide TCM services using the highly effective and evidence-based Wraparound Model. This teaching model goes beyond linking clients to resources by also focusing on individual skills development, family voice and choice, client empowerment, identifying and utilizing strengths, and developing natural support systems to make improvements that are sustainable and allow clients to take over managing basic needs on their own. Our case managers work with clients and their natural support system to identify current strengths and needs, guide clients in setting goals and creating action steps to address these needs.
